Summer Program Registration

To register your child for the Summer Program, please follow these steps:

1. Complete the Child Information Form
   This form only needs to be filled out once per child for the entire summer.

2. Select Your Week
   Choose the week(s) your child will attend from the options below.  You need to fill out a separate form for each week you want to attend.
